"In his capacity as the officer in charge 327 D.I.D., Lieutenant Mazurak achieved outstanding results in his work during 2 Polish Corps Operations in the battle for Bologna. Working under very bad conditions, on damp ground and under Artillery fire, supplying a large number of units. Including a large number of British units, which were more than the normal capacity of the D.I.D., he distinguished himself by his exceptional ability and efficiency.

By his devotion, conscientiousness and energy he greatly contributed to the efficiency of the service throughout the Corps Spring offensive."
